What We’re About

CHFS-CFTSS program is highly focused on addressing the emotional and behavioral health needs of children/adolescents from the early onset of when these challenges develop. Thus, preventing the progression of behavioral health conditions by taking mental health and behavioral health services directly into the community, i.e., home, school, recreation centers, churches, etc. CHFS strives to provide CFTSS services utilizing a cultural lens that understands that the behavioral health needs of communities and families happen, not only within the context of community, but also within a unique cultural context that is defined by the family.

What We Do

The first step in the array of CFTSS services is a full assessment of the child and family’s history, strengths, needs and current functioning as well as mental health screenings. The assessment can be completed in the home or if clinically appropriate, virtually by a licensed mental health professional. Based on the completed assessment, it is determined whether the child is in need of mental health services a treatment plan is developed in conjunction with the family. This plan determines the type of CFTSS services the family will receive. 

Who We Help

Our licensed mental health providers and other experienced providers assist children, youth and their families, with support to build skills for better managing behaviors, specifically addressing the child and family’s needs. We are committed to working alongside the children and families that we serve to strengthen their success in improving their quality of life. By doing so, we are able to identify mental health and/or substance use issues, provide hands-on support and possibly prevent crisis situations.

Referral Process

CFTSS is available to children and youth who are enrolled in Medicaid, a Medicaid Managed Care plan or Child Health Plus. To make a referral to CFTSS or to learn more about the program call 718-497-6090 and ask for the CFTSS program.
