What We’re About
CHFS Family Support Services provides home-based supportive and rehabilitative counseling, as well as case management, to families in Brooklyn and Queens. Our team of dedicated bilingual-bicultural caseworkers deliver these services. Our mission is to support and empower families in ways that enhance child safety and well-being, strengthen family bonds, and foster self-reliance.

What We Do
It is the mission of our family support program to serve, support and strengthen families in ways that will promote child safety and well-being, strong families, and foster self-reliance.
CHFS recognizes that in times of family crisis, a family may find itself vulnerable, and their usual ways of coping and problem solving may not work. Through our case planners, we offer families a series of individualized family-centered services to help them discover and strengthen their problem-solving skills and reduce the risk of family disruption. We do this by drawing from families’ diverse and rich cultural heritage, strengths, family and community supports, and linkages with community resources.
Who We Help
Our free services are available to families with children ranging from birth to 21 years old, pregnant individuals, as well as those at risk of having their children placed in foster care.
Referral Process
Parents interested in accessing our services are encouraged to reach out to us directly or visit any of our locations to begin the application process. Additionally, community providers seeking to refer families can also contact us. To refer please email the following email addresses:
