What We’re About

CHFS Family Treatment & Rehabilitation (FTR) services provide home-based supportive and rehabilitative counseling, as well as case management, to families in Brooklyn and Queens. Our team of dedicated bilingual-bicultural caseworkers deliver a higher level of services to families impacted by mental illness and/or substance abuse. Our mission is to support and empower families in ways that enhance child safety and well-being, strengthen family bonds, and foster self-reliance.

What We Do

The FTR program aims to serve, support, and strengthen families impacted by mental illness and/or substance abuse in ways that will promote child safety and well-being, strong families, and foster self-reliance. This program’s higher level of preventative services are provided to the entire family. During initial stages of engagement the families are visited frequently in the home concentrating on addressing family needs and identifying family strengths. As the family stabilizes and are engaged in resources in the community, visits are gradually reduced to help families continue on the path to self-reliance.

Who We Help

Our free services are available to families with children ranging from birth to 21 years old, pregnant individuals, as well as those at risk of having their children placed in foster care.
