What We Do
Our Health Services interdisciplinary team consists of medical management assistants, LPN’s RN’s, Licensed Behavioral Health Specialists, a Foster Care Medicaid Managed Care Liaison, and a licensed director to oversee the program. Licensed behavioral health practitioners provide mental health assessments, screening, referrals, advocacy and short-term individual, family and group treatment. Other team members arrange for medical appointments and ensure timely follow-up for specialized care. A proactive approach is taken in the medical and dental care of each child ensuring that there is a consistent focus on health and wellness.
Who We Help
Our Health Services program serves children in Foster Care. The program also works closely with foster care staff, foster parents, and birth parents on the day-to-day health and well-being of children and youth in care.
Referral Process
Participants are referred to Health Services (NYSDOH Article 29i) directly from the CHFS Enhanced Family Foster Care program.
